Ontario’s health card gives access to publicly funded health care under the Ontario Health Insurance Plan (OHIP).

  • Who is eligible
    • You must make Ontario your main place of residence.
    • You must be physically present in Ontario for at least 153 days in a 12-month period.
    • You must have an eligible status, such as:
      • Canadian citizen
      • Permanent resident
      • Protected person or convention refugee
      • Valid work permit and working full-time in Ontario
      • Certain applicants for permanent residence
  • No waiting period
    • Ontario currently has no mandatory waiting period for OHIP once you are eligible.
  • Documents you must bring
    • You need three original documents (no photocopies):
      • Proof of status in Canada (e.g., passport, birth certificate, PR card, work permit)
      • Proof you live in Ontario (e.g., lease, utility bill, bank statement, Ontario driver’s licence)
      • Proof of identity with your name and signature (e.g., passport, credit card, student ID)
    • Each document must be from a different category.
  • How to apply
    • Applications must be done in person at a ServiceOntario centre.
    • You will complete a Registration for Ontario Health Insurance Coverage form at the office.
  • After you apply
    • If approved, your health card is mailed to you, usually within a few weeks.
    • You may receive temporary confirmation while waiting for the card.
  • Children and newborns
    • Babies born in Ontario are usually registered by the hospital or midwife.
    • Parents can also apply at ServiceOntario if needed.
  • Renewals and replacements
    • Expired, lost, or damaged cards can be renewed or replaced through ServiceOntario, and in some cases online.
